I, the undersigned individual, consent to my participation in the 5on5 Winter Classic Basketball Tournament and hereby acknowledge, understand, and assume the potential risk(s) of injury involved in my involvement in the Tournament.

I hereby waive and release, indemnify, hold harmless, and forever discharge 5on5 Winter Classic Basketball Tournament, and its agents, employees, team owners, officers, directors, affiliates, sponsors, advertisers, successors and assigns, and all other persons or entities acting on their behalf, including, if applicable, owners and lessors of premises used to conduct activities associated with 5on5 Winter Classic Basketball Tournament, of and from any and all claims, demands, debts, contracts, expenses, causes of action, lawsuits, damages and liability of every kind and nature, whether known or unknown, in law or equity, that I ever had or may have, arising from or in any way related to my involvement in 5on5 Winter Classic Basketball Tournament, including but not limited to negligence, breach of contract, or wrongful contact. By signing this Agreement, it is my intention to surrender and waive any rights to sue or exercise any legal right to seek damages for my participation in 5on5 Winter Classic Basketball Tournament.

I understand that this Agreement shall be binding upon my heirs, agents, personal representatives and assigns.

I hereby certify that I have carefully read the foregoing and acknowledge that I understand and agree to all of the above terms and conditions. I am aware at by receiving this item, I assume all risk and waive and release certain substantial rights I may have or possess, including any and all risk of injury incurred or suffered by me (1) while practicing or playing as a member of 5on5 Winter Classic Basketball Tournament; (2) while serving in a non-playing capacity as a team member during practice or play by other teams or by other players in the 5on5 Winter Classic Basketball Tournament; and (3) while on or upon the premises of any and all of the fields associated with or used by 5on5 Winter Classic Basketball Tournament. I have carefully read this agreement and 5on5 Winter Classic Basketball Tournament Rules & Regulations, and fully understand the contents therein. I am aware the agreement includes a waiver of liability and indemnity agreement.
